New Patient Registration

If you have recently moved into our practice area (please see practice boundary maps below) and wish to register as a patient you will be asked to complete a registration form and given a registration pack. 

Please bring your passport, driving licence or birth certificate plus either a utility bill or statement showing current address in our area or bank or building society statements. If you are unable to provide these identity documents please refer to registration pack for list or contact the reception who will advise what other identification may be acceptable.

For children under the age of 16 whose parent/guardian is registered with the practice/registering at the same time will need to bring either their original full birth certificate or passport. 

The reception staff will tell you about the surgery.

Can you please also bring a list of any current medication you are taking? The repeat slip from your previous surgery will have all this information.
